Cross-layer distributed power control: A repeated games formulation to improve the sum energy-efficiency
The main objective of this work is to improve the energy-efficiency (EE) of a
multiple access channel (MAC) system, through power control, in a distributed
manner. In contrast with many existing works on energy-efficient power control,
which ignore the possible presence of a queue at the transmitter, we consider a
new generalized cross-layer EE metric. This approach is relevant when the
transmitters have a non-zero energy cost even when the radiated power is zero
and takes into account the presence of a finite packet buffer and packet
arrival at the transmitter. As the Nash equilibrium (NE) is an
energy-inefficient solution, the present work aims at overcoming this deficit
by improving the global energy-efficiency. Indeed, as the considered system has
multiple agencies each with their own interest, the performance metric
reflecting the individual interest of each decision maker is the global
energy-efficiency defined then as the sum over individual energy-efficiencies.
Repeated games (RG) are investigated through the study of two dynamic games
(finite RG and discounted RG), whose equilibrium is defined when introducing a
new operating point (OP), Pareto-dominating the NE and relying only on
individual channel state information (CSI). Accordingly, closed-form
expressions of the minimum number of stages of the game for finite RG (FRG) and
the maximum discount factor of the discounted RG (DRG) were established. The
cross-layer model in the RG formulation leads to achieving a shorter minimum
number of stages in the FRG even for higher number of users. In addition, the
social welfare (sum of utilities) in the DRG decreases slightly with the
cross-layer model when the number of users increases while it is reduced
considerably with the Goodman model. Finally, we show that in real systems with
random packet arrivals, the cross-layer power control algorithm outperforms the

L’effet de l’extubation precoce apres chirurgie cardiaque pour la rehabilitation post opératoire
Introduction: La réhabilitation post opératoire en chirurgie cardiaque est basée sur une prise en charge médico-chirurgicale visant à diminuer la durée de séjour hospitalier et les coûts de soins de cette chirurgie à haut risque. L’extubation trachéale précoce, définie par une extubation avant la sixième heure, constitue la pierre angulaire du Fast-tracking. L’objectif de notre étude est d’évaluer dans notre institution la pratique du fasttrack et notamment de l’extubation trachéale précoce en chirurgie cardiaque adulte programmée. Méthodes: Nous avons conduit une étude descriptive incluant tous les patients âgés de plus 18 ans opérés consécutivement d’une chirurgie cardiaque programmée et qui ont été pris en charge en post-opératoire dans l’unité de réanimation post-opératoire du Service de chirurgie thoracique et cardiovasculaire du CHU HABIB BOURGUIBA de SFAX. Les critères d’inclusion des malades: âgés de 18 ans et plus, opérés d’une chirurgie cardiaque programmée, dont la prise en charge post-opératoire s’est faite dans l’unité de réanimation post-opératoire du service de chirurgie cardio-vasculaire et thoracique. Notre protocole d’anesthésie a été standardisé pour tous les patients: Propofol, Rémifentanil, Cisatracrium. Nous avons noté le délai d’extubation en post opératoire et les facteurs qui allongent ce délai d’extubation. Résultats: Nous avons colligé 200 observations de malades consécutifs qui ont bénéficié d’une chirurgie cardiaque programmée. Parmi ces malades 115 étaient opérés pour pontage aorto-coronaire, 79 pour chirurgie valvulaire et 6 pour chirurgie combinée ou autre. Les caractéristiques démographiques sont comparables. En post-opératoire, nous avons pu extuber 152 soit 76% de nos malades avant la sixième heure. Quarante-huit patients n’ont pas pu être extubés avant la sixième heure. Les principales causes de l’échec de l’extubation précoce sont : fortes doses de catécholamines, saignement, arythmie et les troubles neurologiques. Conclusion: Nous avons pu démontrer que la réalisation de la réhabilitation post opératoire était possible dans notre institution et tous les patients bénéficiant d’une chirurgie cardiaque réglée devraient être candidats à l’extubation précoce
Energy-Efficient Spectrum Sharing in Relay-Assisted Cognitive Radio Systems (Invited Paper)
Abstract—This work characterizes an important solution concept of a relevant spectrum game. Two energy-efficient sources communicating with their respective destination compete for an extra channel brought by a relay charging the used bandwidth through a pricing mechanism. This game is shown to possess a unique Nash bargaining solution, exploiting a time-sharing argument. This Pareto-efficient solution can be implemented by using a distributed optimization algorithm for which each transmitter uses a simple gradient-type algorithm and alternately updates its spectrum sharing policy. Typical numerical results show to what extent spectral efficiency can be improved in a system involving selfish energy-efficient sources
Cholécystite aigue gangréneuse alithiasique en postopératoire d’une chirurgie orthopédique: à propos d’un cas
La cholécystite gangreneuse alithiasique postopératoire est une complication grave et sévère, surtout chez les malades hospitalisés en réanimation. Elle survient le plus souvent au décours d'une chirurgie vasculaire ou digestive majeure, d'un polytraumatisme, dans un contexte septique ou dans un contexte de choc. Nous rapportons l'observation d'un homme âgé de 74 ans opéré d'une fracture du col du fémur, au sixième jour postopératoire il a développé un tableau clinique d'une cholécystite aigue dont les explorations radiologiques ont confirmé son caractère alithiasique. We report the case of a 74 year man who underwent surgery for femoral neck fracture. On the sixth postoperative day, he developed acute cholecystitis. Radiological examinations confirmed acalculous cholecystitis.
